The STEPS How to Join the E.Q.O.L. Pooled Special Needs Trust

    The E.Q.O.L. Pooled Special Needs Trust was established with the creation of the Master Trust. All clients or members of the Trust receive this document to read. The Master Trust outlines the purpose and uses for this Trust as well as the restrictions.

  3. SCHEDULE A TELEPHONE ORĀ  OFFICE MEETING WITH THE TRUSTEE
    It is essential that our Pooled Trust members and their legal representatives thoroughly understand how the EQOL Pooled Special Needs Trust can benefit them, to understand the restrictions and guidelines which apply. During this conversation, we hope to answer all questions about the Trust. Our Trustees cannot offer legal advice. Any questions about how a Pooled Special Needs Trust works with your government benefits, or other legal and tax questions must be directed to your attorney or accountant.

  4. DOWNLOAD AND COMPLETE THE JOINDER AGREEMENT
    The E.Q.O.L. Joinder Agreement is the application for membership. An E.Q.O.L. Trustee can be available to answer questions as you fill it out. There are several areas where important choices must be made. The Joinder Agreement should be signed, witnessed and notarized by a Florida Notary. If the member or their legal representative would like to come to the E.Q.O.L. office to sign the document with witnesses and notary present, an appointment can be scheduled.

    Once the Joinder Agreement is completed and executed, it can be faxed or mailed to our offices. If the document has been executed by a Power of Attorney or Guardian, copies of the legal documents granting that authority must be attached to the joinder Agreement.

  5. E.Q.O.L. TRUSTEES WILL SET UP THE ACCOUNT WITHIN A FEW DAYS
    Members or their legal representatives will be notified how to make the first deposit of funds, either by check or wire transfer.

  6. WELCOME PACKET
    Once the Pooled Special Needs Trust sub-account is set up and funded, the member or their legal representatives will receive a Welcome packet which includes a receipt of their deposit, information about their account number and a customized Trust Distribution Form, which will be used to request payments to vendors.

  7. CUSTOMER SERVICE
    Just like your bank, E.Q.O.L. maintains typical business hours of Monday through Friday from 9am-5pm except during all banking or religious holidays.
